Independence at home

Specialist one-to-one support at home for complex needs—including airway management, enteral nutrition and epilepsy protocols—delivered by carers trained to your competencies and overseen by registered nurses. We fit care around your routines and goals so independence and safety go hand in hand.

How we do it

Person-Centred Plans

Goals, preferences, clinical needs and risks shape each plan and review.

Continuity of Care

We aim to build a small, consistent team around each person.

Competency-Based Support

Training and competency checks are matched to the tasks in each care plan.

1. Quality & Clinical Governance

Clinical oversight is matched to each package. Reviews, incident learning, medicine checks and task-specific competencies are built into governance, with escalation arrangements recorded in the care plan.

2. Mobilisation & Continuity

We aim to build a small, consistent team around each person. Staff complete the training and competency checks required for their role before delivering the relevant clinical tasks.

3. Measuring Outcomes

From day one we agree clear goals (comfort, safety, community access, reduced hospital use). We track them monthly, review with your clinicians and adapt the plan quickly—keeping care person-centred and clinically sound.
